ABSTRACT

OBJECTIVE: to test the factorial structure, reliability and convergent validity of the Functional Health Pattern Assessment Screening Tool - Modified Brazilian Version. METHOD: this was a psychometric evaluation of the Functional Health Pattern Assessment Screening Tool - Modified Brazilian Version. Seven hundred and seventeen participants answered the data collection instrument consisting of two parts. Part I included a structured questionnaire to collect sociodemographic data and the participants' perceptions and satisfaction with their current health status. Part II consisted of the tool being tested. The internal structure was assessed using Confirmatory Factor Analysis. Convergent validity was evaluated by the correlation of the tool scores with the rates corresponding to self-perception and satisfaction with current health status. Reliability was assessed using Cronbach's alpha. RESULTS: the Confirmatory Factor Analysis confirmed a three-factor solution. The factor loadings were significant and varied from 0.16 to 0.75; the fit indices suggested moderate fit of the model. Internal consistency for all three components varied between 0.779 and 0.919. CONCLUSION: the findings suggest that the tool is valid and reliable to be used in the Brazilian population, although caution is recommended when interpreting the results due to the moderate fit of the model. Objetivo: someter a prueba la estructura factorial, confiabilidad y validez convergente del instrumento Functional Health Pattern Assessment Screening Tool - Versión modificada para Brasil. Método: evaluación psicométrica del instrumento Functional Health Pattern Assessment Screening Tool - Versión modificada para Brasil. Setecientos diecisiete participantes respondieron el instrumento de recolección de datos, compuesto por dos partes. La Parte I incluyó un cuestionario estructurado para recopilar datos sociodemográficos y las percepciones y el nivel de satisfacción de los participantes con respecto a su estado de salud actual. La Parte II consistió en la herramienta sometida a prueba. La estructura interna se evaluó empleando Análisis Factorial Confirmatorio. La validez interna se evaluó por medio de la correlación entre las puntuaciones obtenidas en la herramienta y los índices correspondientes a los niveles de autopercepción y satisfacción con respecto al estado de salud actual. La confiabilidad se evaluó utilizando el coeficiente alfa de Cronbach. Resultados: el Análisis Factorial Confirmatorio confirmó una solución con tres factores. Las cargas factoriales fueron significativas y variaron entre 0,16 y 0.75; los índices de ajuste sugirieron ajuste moderado del modelo. La consistencia interna correspondiente a los tres componentes varió entre 0,779 y 0,919. Conclusión: los hallazgos sugieren que la herramienta es válida y confiable para ser usada en la población de Brasil, aunque se recomienda interpretar los resultados con precaución debido al moderado ajuste del modelo.

Setecentos e dezessete participantes responderam os itens do instrumento de coleta de dados composto por duas partes. A Parte I incluiu um questionário estruturado contendo dados sociodemográficos e a percepção e satisfação dos participantes com seu estado de saúde atual. A Parte II consistiu no instrumento testado. A estrutura interna foi avaliada por meio de Análise Fatorial Confirmatória. A validade convergente foi avaliada pela correlação dos escores do instrumento com os índices correspondentes à autopercepção e à satisfação com o estado de saúde atual. A confiabilidade foi avaliada pelo alfa de Cronbach. Resultados: a Análise Fatorial Confirmatória confirmou uma solução de três fatores. As cargas fatoriais foram significativas e variaram de 0,16 a 0,75; os índices de ajuste sugeriram ajuste moderado do modelo. A consistência interna dos três componentes variou entre 0,779 e 0,919. Conclusión: os achados sugerem que o instrumento é válido e confiável para ser utilizado na população brasileira, embora seja recomendada cautela na interpretação dos resultados devido ao ajuste moderado do modelo.

METHOD: a randomized, parallel and double-blind (researchers and outcome evaluators) clinical trial, carried out with undergraduate Nursing students. Validated case studies were applied in two phases to identify the patient's Nursing diagnosis/problem, etiology and clues, using the Guide with the intervention group in the second phase. The outcomes - diagnostic and etiological accuracy and number of clues - were evaluated using validated rubrics. Descriptive statistics were used to analyze demographic data; Fisher's exact test for similarities in prior education and confidence; Mann-Whitney's test for age; and non-parametric ANOVA test in the evaluation of the hypothesis of differences in performance. RESULTS: final sample composed of 24 students in the control group and 27 in the intervention group; no difference as to gender, age and schooling. There was a difference in diagnostic (p=0.041) and etiological (p=0.0351) accuracy in the intervention group, showing a negative effect of using the Guide. CONCLUSION: the one-time self-instruction was not effective in impacting the diagnostic accuracy of students solving case studies. Repeated application of the Guide as a teaching tool can be effective in improving such outcome. REBEC: RBR-4bhr78.

ABSTRACT

PURPOSE: To assess the validity of a Written Clinical Reasoning Prompt (WCRP) to help nursing students' clinical reasoning (CR). METHOD: This is a methodological study of translation and cross-cultural adaptation into Brazilian Portuguese of instruments that aim to promote CR improvement, composed of (1) WCRP; (2) two case studies; (3) a questionnaire about students' perceptions during decision-making in case studies; (4) a scoring rubric for correcting case studies. For translation and cross-cultural adaptation, stages 1-8 of the Research Diagnostic Criteria for Temporomandibular Disorders (RDC/TMD) Consortium Network were adopted. Agreement values among experts >80% and content validity coefficient (CVC) > 0.8 were considered satisfactory. For the pretest, a randomized clinical trial was carried out with 24 nursing students (intervention group, n = 14, using the WCRP to solve case studies; control group, n = 10, without using the WCRP). FINDINGS: The WCRP was translated and adapted into Brazilian Portuguese, requiring minimal adjustments to obtain agreement among the judges above 80% and CVC above 0.80. Regarding face validation, an adequate agreement was obtained in the assessment by students. There was no difference in the accuracy of nursing diagnoses between the intervention and control groups in the pretest. CONCLUSION: The WCRP was translated and adapted into Brazilian Portuguese and had adequate face and content validity estimates; however, there was no association with improving nursing students' diagnostic accuracy. IMPLICATIONS FOR NURSING PRACTICE: The translated and adapted versions of all instruments into Brazilian Portuguese had adequate evidence of content and face validity. The use of WCRP was not associated with a significant improvement in nursing students' diagnostic accuracy. New studies with larger samples, a sample power of at least 80%, and a level of significance of 5% are needed.

Resumo Objetivo avaliar o efeito do Guia Autoinstrucional para Raciocínio Clínico na acurácia diagnóstica de estudantes de Bacharelado em Enfermagem. Método ensaio clínico randomizado, paralelo, duplo cego (pesquisadores e avaliadores dos desfechos), realizado com graduandos de Enfermagem. Aplicaram-se, em duas fases, estudos de caso validados para a identificação de diagnóstico de enfermagem/problema do paciente, etiologia e pistas, sendo utilizado o Guia com o grupo intervenção na segunda fase. Os desfechos - acurácia diagnóstica, etiológica e número de pistas - foram avaliados por meio de rubricas validadas. Para análise, utilizou-se estatística descritiva para dados demográficos; teste exato de Fisher para similaridades na educação prévia e confiança; teste de Mann-Whitney para idade; teste ANOVA não paramétrico na avaliação da hipótese de diferenças no desempenho. Resultados amostra final composta por 24 estudantes no grupo controle e 27 no intervenção; sem diferença quanto ao sexo, idade e educação. Houve diferença para acurácia diagnóstica (p=0,041) e etiológica (p=0,0351) no grupo intervenção, mostrando efeito negativo da utilização do Guia. Conclusão a autoinstrução implementada uma única vez não foi efetiva em impactar a acurácia diagnóstica de estudantes solucionando estudos de caso. A aplicação repetida do Guia como ferramenta didática pode ser efetiva para melhorar tal desfecho. REBEC: RBR-4bhr78.

Método ensayo clínico aleatorizado, paralelo, doble ciego (investigadores y evaluadores de resultados), realizado con estudiantes de la carrera de enfermería. Se aplicaron estudios de casos validados en dos fases para identificar el diagnóstico de enfermería/problema del paciente, etiología y pistas, utilizando la Guía con el grupo experimental en la segunda fase. Los resultados (precisión diagnóstica, etiológica y número de pistas) se evaluaron utilizando rúbricas validadas. Para el análisis se utilizó estadística descriptiva para datos demográficos; prueba exacta de Fisher para similitudes en educación previa y confianza; prueba de Mann-Whitney para la edad; prueba ANOVA no paramétrica en la evaluación de la hipótesis de diferencias en el desempeño. Resultados muestra final compuesta por 24 estudiantes en el grupo control y 27 en el grupo experimental; no había diferencias en cuanto al sexo, la edad y educación. Hubo diferencia en la precisión diagnóstica (p=0,041) y etiológica (p=0,0351) en el grupo experimental, que mostraron un efecto negativo del uso de la Guía. Conclusión la autoinstrucción implementada por única vez no fue efectiva para generar un impacto en la precisión diagnóstica de los estudiantes que resolvieron los estudios de casos. ABSTRACT

PURPOSE: To test the effects of clinical reasoning prompts on students' clinical judgment of a written case study. METHODS: An experimental pre- and posttest study with second semester nursing students (N = 163). FINDINGS: The intervention was insufficient to significantly improve clinical judgment. Students identified that the prompts would help them "narrow… down the problem" and "slow… the decision-making process" to improve analysis. The most accurate patient problem was identified by 28% of students in pretest and 35% in posttest. CONCLUSIONS: This study provides evidence of variations in nursing students' clinical judgment and students' desire to use decision-making algorithms. NURSING IMPLICATIONS: Nurse educators should provide students with additional education and practice to identify and solve these types of problems.

ABSTRACT

OBJECTIVES: To summarize evidence in the literature on the predictors of insomnia in adults and to determine correspondences with diagnostic indicators of the NANDA-I diagnosis Insomnia. METHODS: An integrative review performed in Pubmed, Virtual Health Library and CINAHL. Forty-eight articles published in Portuguese, English or Spanish from 2011 to 2018 were included. An analysis of correspondence between the predictors and the NANDA-I related factors and associated conditions for Insomnia was performed. RESULTS: There was a correspondence of the predictors found in this review with NANDA-I related factors and associated conditions, except for grieving and frequent naps during the day. Smoking, caffeine intake, dysfunctional sleep beliefs, obesity and caregiver role strain are possible new related factors; chronic illness is a possible new associated condition and individuals going through changes in marital status, economically disadvantaged, female gender, increasing age and night shift worker are possible new at-risk populations. CONCLUSION: The predictors of insomnia that had a correspondence with the NANDA-I elements can support the evidence base of the nursing diagnosis. The predictors found without a correspondence with the diagnosis can be considered for inclusion in the NANDA-I classification, thereby supporting the clinical reasoning of nurses and students.

Resumo Objetivo Investigar as evidências científicas sobre as úlceras de pernas como sequela da hanseníase. Métodos Revisão Integrativa da Literatura (RIL). Para identificação do tema e questão de pesquisa, utilizou-se a estratégia PICo, na sequencia procedeu-se a busca nas bases de dados: Index to Nursing and Allied Health Literature (Cinahl), Literatura Latino-americana e do Caribe em Ciências da Saúde (Lilacs), Banco de dados em Enfermagem (BDENF), EMBASE (Elsevier), PUBMED (National Library of Medicine), SCOPUS (Elsevier), Web of Science (Clarivate Analytics), selecionados os descritores específicos para cada uma delas e elaboradas as respectivas estratégias de busca. Os idiomas considerados foram: inglês, português e espanhol. O período da RIL foi indeterminado. Para a seleção dos estudos, utilizou-se a ferramenta Ryyan que permitiu dois dos autores independentemente incluir, excluir, ou ficar indeciso, as discordâncias foram solucionadas por um terceiro. Os artigos selecionados foram classificados de acordo com a Agency for Healthcare Research and Quality (AHRQ). E para a interpretação dos resultados os artigos foram agrupados por semelhança e categorizados da seguinte maneira: autor, ano país, nível de evidência, objetivos e resultados, conclusão e recomendação. Resultados Identificou-se 415 estudos e 10 foram incluídos na revisão. O resultado principal obtido foi o tratamento tópico da úlcera, as vulnerabilidades e os determinantes que assolam os acometidos pela hanseníase não foram estudados. Conclusão Não há fortes evidências sobre as terapias para o tratamento de úlceras de pernas decorrentes da hanseníase que apoiem a diminuição de sequelas incapacitantes ocasionadas pela doença perpetuando o estigma e a desigualdade social.

Para la identificación del tema y pregunta de la investigación, se utilizó la estrategia PICo, luego se procedió a la búsqueda en las bases de datos: Index to Nursing and Allied Health Literature (Cinahl), Literatura Latinoamericana y del Caribe en Ciencias de la Salud (Lilacs), Banco de dados em Enfermagem (BDENF), EMBASE (Elsevier), PUBMED (National Library of Medicine), SCOPUS (Elsevier), Web of Science (Clarivate Analytics), se seleccionaron los descriptores específicos para cada una y se elaboraron las respectivas estrategias de búsqueda. Se consideraron los idiomas inglés, portugués y español. El período de la RIL fue indeterminado. Para la selección de los estudios, se utilizó la herramienta Ryyan, que permitió dos de los autores independientemente incluir, excluir o estar indeciso, las discrepancias fueron resueltas por un tercero. Los artículos seleccionados se clasificaron según la Agency for Healthcare Research and Quality (AHRQ). Y para la interpretación de los resultados, los artículos se agruparon por semejanza y se categorizaron de la siguiente manera: autor, año, país, nivel de evidencia, objetivos y resultados, conclusión y recomendación. Resultados se identificaron 415 estudios y se incluyeron 10 en la revisión. El resultado principal obtenido fue el tratamiento tópico de la úlcera, las vulnerabilidades y los determinantes que asolaron a los afectados por la lepra no fueron estudiados. Conclusión no hay fuertes evidencias sobre las terapias para el tratamiento de úlceras de piernas resultantes de la lepra que justifiquen la reducción de secuelas incapacitantes provocadas por la enfermedad, perpetuando el estigma y la desigualdad social.


Abstract Objective To investigate the scientific evidence on leg ulcers as leprosy sequel. Methods This is an Integrative Literature Review (ILR). In order to identify the theme and research question, the PICo strategy was used, in the sequence the search was carried out in Index to Nursing and Allied Health Literature (CINAHL), Latin American & Caribbean Literature in Health Sciences (LILACS), (BDENF), EMBASE (Elsevier), PUBMED (National Library of Medicine), SCOPUS (Elsevier), Web of Science (Clarivate Analytics) databases. Specific descriptors were selected for each of them and respective search strategies were created. The languages considered were English, Portuguese and Spanish. The ILR period was indeterminate. For the study selection, the Ryyan tool was used and allowed two of the authors to independently include, exclude or remain undecided, disagreements were solved by a third party. The selected articles have been ranked according to the Agency for Healthcare Research and Quality (AHRQ). And for interpretation of results, the articles were grouped by similarity and categorized into author, country year, level of evidence, objectives and results, conclusion and recommendation. Results 415 studies were identified and 10 were included in the review. The main outcome was the topical treatment of ulcers, the vulnerabilities and the determinants that afflict those affected by leprosy were not studied. Conclusion There is no strong evidence on therapies for the treatment of leprosy-induced leg ulcers that support the reduction of disabling sequel caused by disease perpetuating stigma and social inequality.

Resumo Objetivo Verificar se o medo de cair é fator de risco em pessoas idosas que vivem na comunidade. Métodos Foi realizada uma revisão sistemática com metanálise baseada na Preferred Reporting Items for Systematic Reviews and MetaAnalyses. Utilizou-se a estratégia de busca PECOS: Paciente - pessoa idosa com 60 anos, ou mais, que vive em comunidade com história pregressa de quedas; Exposição- medo de cair, Comparação -grupo sem medo de cair, o "Outcome" - o desfecho queda e o "Studies"- foram incluídos os estudos observacionais comparativos. As buscas foram realizadas em maio de 2018 nos seguintes bancos de dados eletrônicos CINAHL, Medline, Cochrane, Embase, Lilacs, PsycINFO e PEDro, por meio dos seguinte descritores: "aged", "elderly", older adults, fear, fear of falling, accidental falls, fall, fallls. Foram também realizadas buscas de referências cruzadas e literatura cinzenta. Dois revisores realizaram a identificação, seleção, elegibilidade e inclusão dos estudos de maneira independente. A qualidade metodológica dos estudos foram efetuada pela aplicação do instrumento STROBE. Para a metanálise, utilizou-se o Programa ReviewMananger 5.3®. Resultados De 4.891 publicações, cinco estudos possibilitaram a metanálise com 3.112 idosos. Evidenciou-se uma chance de queda de 12,15 vezes maior para o grupo de idosos com medo de cair. Conclusão O medo de cair foi identificado como fator de risco de queda na população idosa que vive na comunidade e que possui história pregressa de queda, torna-se necessário a investigação pelos profissionais da ara da saúde afim de estabelecer medidas preventivas.


Resumen Objetivo verificar si el miedo de caer es factor de riesgo en personas mayores que viven en la comunidad. Métodos se realizó una revisión sistemática con metanálisis basada en la Preferred Reporting Items for Systematic Reviews and MetaAnalyses. Se utilizó la estrategia de búsqueda PECOS: Paciente (ancianos con 60 años o más, que viven en comunidad con historial anterior de caídas); Exposición (miedo de caer); Comparación (grupo sin miedo de caer); "Outcome" (el desenlace de la caída) y "Studies" (fueron incluidos los estudios observacionales comparativos). Las búsquedas fueron realizadas en mayo de 2018 en los siguientes bancos de datos electrónicos CINAHL, Medline, Cochrane, Embase, Lilacs, PsycINFO y PEDro, por medio de los siguientes descriptores: "aged", "elderly", older adults, fear, fear of falling, accidental falls, fall, fallls. También se realizaron búsquedas de referencias cruzadas y literatura gris. Dos revisores llevaron a cabo la identificación, selección, elegibilidad e inclusión de los estudios de manera independiente. La calidad metodológica de los estudios fue efectuada mediante la aplicación del instrumento STROBE. Para el metanálisis, se utilizó el programa ReviewMananger 5.3®. Resultados de 4.891 publicaciones, 5 estudios posibilitaron el metanálisis con 3.112 ancianos. Se observó una posibilidad de caída 12,15 veces mayor en el grupo de ancianos con miedo a caer. Conclusión el miedo a caer fue identificado como factor de riesgo de caída en personas mayores que viven en la comunidad y que poseen historial anterior de caída. Resulta necesaria la investigación por parte de profesionales del área de la salud a fin de establecer medidas preventivas.


Abstract Objective to verify if fear of falling is a risk factor in older people living in the community. Methods A systematic review was conducted with a meta-analysis based on the Preferred Reporting Items for Systematic Reviews and MetaAnalyses. The PECOS search strategy was used: Patient - elderly person aged 60 years or above, living in a community with a previous history of falls; Exposure - Fear of falling, Comparison - group without fear of falling, "Outcome" - fall outcome and "Studies" - included comparative observational studies. The searches were conducted in May 2018 in the following electronic databases CINAHL, Medline, Cochrane, Embase, Lilacs, PsycINFO and PEDro, through the following descriptors: "aged", "elderly", older adults, fear, fear of falling, accidental falls, fall, falls. Cross-references and gray literature were also searched. Two reviewers independently performed the identification, selection, eligibility and inclusion of the studies. The methodological quality of the studies was carried out by applying the STROBE tool. For the meta-analysis, the Review Mananger 5.3 Program was used®. Results Of 4,891 publications, five studies enabled a meta-analysis with 3,112 elderly. There was a chance of a fall of 12.15 times higher for the group of elderly people with fear of falling. Conclusion Fear of falling was identified as a risk factor for falling in the elderly population that lives in the community and that has a history of falling, it is necessary the investigation by health professionals in order to establish preventive measures.
